L'usage de cannabis et l'insécurité routière: Étude par questionnaires et observations sur simulateur de conduite

Abstract (summary)

Cannabis is the illegal drug most often found in the bodily fluids of drivers who are seriously injured or die in road collisions. However, the studies that estimated the road risk associated with cannabis use yielded contradictory results. As such, the impact of cannabis use on the risk of collision has not yet been determined. This can be partly attributed to the fact that confounding variables, namely dangerous driving, are rarely controlled for in studies. "At-risk" automobile driving can result from various motivations, including deliberate risk-taking or expressing one's anger on the road. It is thus necessary to take into account the multi-dimensional nature of dangerous driving when studying it. The main objective of this thesis is to verify the relationship between cannabis use and unsafe driving.

In this line of thinking, article 1 discusses the French adaptation of a questionnaire designed to measure dangerous driving—the Dula Dangerous Driving Index (DDDI). A preliminary validation was performed on seventy-five male drivers. The drivers completed two self-administered questionnaires in addition to tasks in a driving simulator, which provided an objective measure of risky driving behaviours. The results indicated that the French adaptation of the DDDI contains satisfactory psychometric qualities and that it constitutes a useful tool in identifying dangerous driving behaviours, that is, risky driving, aggressive driving and negative emotions and cognitions experienced on the road.

Article 2 was conducted based on data collected from a sample of 260 male and female drivers of varying ages and occupation. The drivers completed a battery of self-report questionnaires. The results indicate that cannabis use is positively associated with risk-taking on the road and aggressive driving, beyond age, gender or driving exposure. Furthermore, cannabis use is associated with an increased risk of having received a traffic conviction. Cannabis use is also linked to an increased risk of collision in drivers over the age of 35.

The results reported in the third article demonstrate an association between self-reported driving under the influence of cannabis and risky driving, which was measured based on questionnaires and behaviours observed in the driving simulator. Moreover, the results indicate that driving under the influence of cannabis is positively and independently associated with sensation-seeking and impulsivity. Finally, a trend suggests that driving under the influence of cannabis is associated with a heightened risk of being involved in a road accident, after controlling for age, driving exposure, driving under the influence of alcohol and dangerous driving.

In summary, this thesis shows that cannabis use and driving after cannabis use are indicators of unsafe driving. Furthermore, the individuals who reported having driven under the influence of cannabis appeared to adopt several dangerous driving behaviours, which partially supports the driving problem behaviour theory. Lastly, this thesis provides additional evidence of the link between driving after cannabis use and an elevated risk of collision.

Le cannabis est la drogue illegale la plus souvent retrouvee dans les fluides corporels des conducteurs blesses grievement ou decedes dans une collision. Or, les resultats des etudes qui ont estime le risque routier associe au cannabis se sont souvent averes contradictoires. Ainsi, l'apport independant du cannabis sur le risque de collision n'est pas encore determine. Ceci decoule en partie de limites d'ordre methodologique et de facteurs de confusion rarement controles dans les etudes, notamment la conduite dangereuse. L'insecurite routiere peut sous-tendre plusieurs motivations, dont prendre des risques de maniere deliberee ou manifester sa colere sur la route. II est done necessaire d'etudier la conduite dangereuse en tenant compte de sa nature multidimensionnelle. Le principal objectif de cette these est de documenter les interactions entre l'usage de cannabis et l'insecurite routiere. 

C'est dans cette optique, l'article 1 traite de l'adaptation francaise d'un questionnaire permettant de mesurer la conduite automobile dangereuse, le Dula Dangerous Driving Index (DDDI). Une validation preliminaire a ete effectuee sur soixante-quinze conducteurs masculins. Ces derniers ont complete des questionnaires auto-administres et des taches sur un simulateur de conduite mesurant de maniere objective les comportements routiers « a risque ». Les resultats indiquent que l'adaptation francaise du DDDI detient des qualites psychometriques satisfaisantes et constitue un outil utile afin de distinguer la conduite risquee, de l'agressivite au volant et des emotions et cognitions negatives vecues sur la route.

L'article 2 a ete realise a partir des donnees recueillies aupres d'un echantillon de 260 conducteurs diversifies en termes d'age, de genre et d'occupation. Ces derniers ont rempli une batterie de questionnaires auto-rapportes. Les resultats indiquent que 1'usage de cannabis est associe de maniere positive a la prise de risque sur la route et a l'agressivite au volant, et ce, au-dela de l'age, du genre et de l'exposition a la conduite. De plus, la consommation de cannabis est associee a une augmentation du risque d'avoir recu une contravention decoulant d'une infraction routiere. L'usage de cannabis est egalement lie a un risque accru de collision chez les consommateurs ages de 35 ans et plus. 

Les resultats rapportes dans le troisieme article mettent en evidence une association entre la conduite sous 1'influence du cannabis auto-rapportee et la conduite risquee mesuree a partir de questionnaires et de comportements observes en conduite simulee. En outre, les resultats indiquent que la conduite sous 1'influence du cannabis est associee positivement et de maniere independante a la recherche de sensations fortes et a l'impulsivite. Finalement, une tendance suggere que la conduite sous l'influence du cannabis est associee a un risque accru d'etre implique dans un accident de la route en controlant pour l'age, l'exposition a la conduite, la conduite sous l'influence de l'alcool et la conduite automobile dangereuse. 

En resume, il se degage de cette these que l'usage de cannabis et la conduite apres la consommation de cannabis sont des indicateurs d'insecurite routiere. De plus, les individus qui rapportent avoir conduit sous 1'influence de cette substance semblent adopter plusieurs comportements routiers dangereux, ce qui appuie partiellement la theorie du syndrome de comportements routiers deviants. Finalement cette these apporte une preuve supplemental du lien entre la conduite apres l'usage de cannabis et le surrisque de collision.
