La dynamique des savoirs dans un service bénévole de soins palliatifs à domicile

Ce rapport de recherche rend compte d'une étude ethnographique ayant comporté une période de plus de deux ans d'observation participante dans un organisme bénévole de soins palliatifs à domicile suivie d'une autre période de plus de deux ans d'études documentaires et d'analyse interprétative. La participation active aux activités du service et la réalisation de 38 entrevues auprès de malades et de membres de leur famille, de quelques accompagnatrices soignantes, de Lise B., la fondatrice-directrice de l'organisme et d'autres collaboratrices et collaborateurs du service ont fourni le matériel d'analyse qui a ensuite été interprété à la lumière de modèles actuels en éducation et ce, après une première synthèse des données recueillies et la rédaction d'une chronique des apprentissages et de l'usage des savoirs des différents partenaires du service.

Le séjour de recherche s'est déroulé en trois phases. La familiarisation avec le service s'est faite pendant une période de quelques mois en 1995; les visites à domicile effectuées en compagnie de Lise B. ont permis la rencontre de plusieurs malades et de membres de leur famille. Ensuite, pendant l'année scolaire 1995–1996, la chercheure a participé au programme de formation théorique et pratique des accompagnatrices soignantes bénévoles. Finalement, une période d'implication particulièrement intense correspond à la dégradation de l'état de santé de Lise B., à l'assistance dans la rédaction de son testament professionnel et à une forme d'accompagnement personnel et professionnel jusqu'au jour de son décès en décembre 1996.

Ce travail de recherche était né du désir de mieux comprendre les possibilités de formation en soins de santé de personnes sans formation scientifique préalable. L'investigation s'est considérablement élargie au contact des individus reliés au service de soins palliatifs et tout particulièrement par la rencontre de la fondatrice du service, madame Lise B. Dans la foulée du virage ambulatoire en soins de santé, l'expérience vécue pendant plus de 15 ans à St-B. semble un modèle exemplaire sur lequel il serait pertinent de réfléchir pour en dégager des avenues permettant un meilleur soin aux malades, particulièrement aux malades en phase terminale. This research report describes an ethnographic study that included a period of more than two years of participant observation in a voluntary home palliative care organization followed by another period of more than two years of documentary studies and interpretative analysis. The active participation in the activities of the service and the realization of 38 interviews with patients and members of their family, a few caregivers, Lise B., the founder-director of the organization and other collaborators of the service provided the analysis material which was then interpreted in the light of current models in education and this, after an initial synthesis of the data collected and the writing of a chronicle of learning and the use of knowledge by the various partners of the service.

The research stay took place in three phases. Familiarization with the service took place over a period of a few months in 1995; the home visits made in the company of Lise B. made it possible to meet several patients and members of their families. Then, during the 1995–1996 school year, the researcher took part in the theoretical and practical training program for volunteer caregivers. Finally, a particularly intense period of involvement corresponds to the deterioration of Lise B.'s state of health, to assistance in drafting her professional will and to a form of personal and professional support until the day of his death in December 1996.

This research work was born from the desire to better understand the possibilities of training in health care for people without prior scientific training. The investigation was broadened considerably through contact with individuals connected to the palliative care service and especially through meeting the founder of the service, Mrs. Lise B. In the wake of the ambulatory shift in health care, the experience lived during more than 15 years in St-B. seems an exemplary model on which it would be relevant to reflect in order to identify avenues allowing better care for the sick, particularly the terminally ill.
